Dear Project Officer; In order for your product to start up in a timely and healthy manner, please check the "Yes" or "No" boxes by examining the questions below. Make sure to review the assembly diagram in the user and assembly manual or which you will request from ACS. If this form is not delivered to ACS Technical Service at least 7 working days before your start up date, the start up process does not start. When our team arrives at your worksite, if the start up conditions are not ready or missing, the process of start up for the second time will be made with a fee. If you need extra information about this form or your device, ACS will gladly support you. Best wishes for a successful business together.
